According to Reveal, the 25-year-old made the decision after Tweed appeared on television show Jeremy Kyle to discuss his marriage to the late Jady Goody.
Goody died in 2009 after losing her fight with cancer.
In the interview on Wednesday, Tweed spoke about how he cared for his wife as her condition deteriorated, and how he was unable to cope in her final moments.
He admitted: "The doctors said, 'You'll be able to tell when it's going to happen because she'll struggle for her breath'. It gradually got worse and worse and I slept on the floor every night.
"The last night [her breathing] was just getting heavier and heavier. Her whole body was moving because she was just fighting to breathe."
An emotional Tweed continued: "I just couldn't sit there anymore. I just kissed her and walked out. Then it went silent after a couple of minutes."
Following the 25-year-old's revelations, Ms Hayes took to Twitter and said the couple had now decided to end their three-year relationship.
The couple have dated on and off over the three years, with Hayes having previously criticising Tweed for continuing to mourn his late wife.
In a series of calm and respectful tweets, Hayes wrote: "Just to clarify re: Jeremy Kyle interview today. I think Jack did very well and I admire him for his commitment to Jade in the later part of her life.
"It is overwhelmingly clear to me we shouldn't have ever entered a relationship when he was still so badly grieving...this fact was never made clear to me and was always denied by him regardless of the support I offered to him.
"Lately, the toll of this has caused us to come to the decision that him staying with us when he is not yet ready to move on from the love of his life is simply unfair to all involved."
The mum-of-one, whose son Blakely is from a previous relationship, added: "I wish Jack all the best for the future and hope that he manages to learn to deal with his grief and be happy finally."
