Fallon, who also worked as her Love's manager, announced the news on Wednesday.
In a statement, she said: "Words cannot express my sadness today to have to tell you that Abz and I are separating after 5 years together.
"I'd like to thank all the #5ivers and #Ateam for the love and kindness you've me over the years and wish you all the best in your futures. Vicky."
The news comes just days after it was revealed Abz would not be joining his bandmates on tour due to "personal reasons". It is not clear if the two situations are related.
The singer, who took part in last year's reality television show, The Big Reunion, he revealed to viewers how the couple met. He said the couple had met after the boyband went their separate ways and he was going through a difficult time in his life as he struggled with drink and drugs.
Speaking openly, he told Fallon and the viewers watching the show: "She saved my life in my eyes because I was on a road to self-destruction."
"It was just before I met you that I was on my way down I guess."
His partner replied: "It was like there was a bit of a demon...something dark around you. It was literally like Jekyll and Hyde."
